• Except for services that you can self-refer to, your Primary Care Provider (PCP) will provide or direct all of your medical care. If your condition requires the care of a specialist or requires a hospital stay, your PCP will refer you to an appropriate CeltiCare provider

How to Use This Directory:

Each provider is listed alphabetically by specialty and location.  Insured in each provider entry:

*To check the most current board certification status you may visit the American Board of Medical Specialties (ABMS) website at www.abms.org.  Click on “Is Your Doctor Certified?” and Register or Log In and follow the instructions.
