CeltiCare gives you more ways to pay for your healthcare with our CentAccount® program. We give you money for completing easy, healthy behaviors! Use this money to pay for copays or even certain over-the-counter items.

How to Start Earning Money:

Healthy Behavior Amount Earned Reward Information
1. Complete the Health Risk Assessment (HRA) form. $50 1 Reward per lifetime.
2. Complete a “Wellness” visit with your PCP within the first 90 days of becoming a CeltiCare member.* $50 1 Reward per lifetime.
3. Complete an annual physical exam with your CeltiCare PCP. ** $50 1 Reward per benefit year (visits must be at least 275 days apart).
* Initial PCP visit and annual physical cannot be completed in the same office visit.
** Annual physical exam cannot take place within the first 90 days of membership.
*** If you are an existing patient with your PCP, call Member Services Department for details on the “wellness” visit credit.

You can start earning money as soon as you become a CeltiCare member! After you complete one of the healthy behaviors a personalized, CentAccount Prepaid MasterCard® will be mailed to you at your home address. Additional money will be added to the card when you complete the next healthy behavior. You can earn up to $150 on your CentAccount Prepaid Master Card in your first year of CeltiCare membership by completing the following healthy behaviors:

1.) Complete the Health Risk Assessment (HRA) form.

 

    HRA forms can be submitted electronically through the Member Portal (accessed under Current Members tab on this website), or by mailing in the form located in your CeltiCare Welcome Packet to:

2.) Complete a “Wellness” visit with a Primary Care Physician (PCP) in the CeltiCare Network within your first 90 days of becoming a member.

 

    Locating a PCP within the CeltiCare Network is simple, all you have to do is select the Find a Doctor tab above and click on Start CeltiCare Provider Search to begin searching for PCP’s in your area.

3.) Complete an annual physical with a CeltiCare in-network PCP within the first year of becoming a member of CeltiCare.*

 

Using Your CentAccount Prepaid Mastercard:

Your CentAccount Prepaid card can be used at CVS, Wal-Mart, Stop n’ Shop and lots of other stores.  You can also use the money on your CentAccount Prepaid card at doctor’s offices that accept MasterCard®**.  You can use the funds in your account towards many benefits, including but not limited to:

  • Eligible expenses at your healthcare provider’s office
  • Copayments
  • Denture adhesives, repair and cleansers
  • Diabetes testing & aids
  • Un-medicated condoms
  • Diagnostic products
    • Thermometers
    • Blood pressure monitors
    • Cholesterol testing
  • Ear care
  • Elastic bandages
  • Braces
  • Eye care (including contact lens care)
  • Family planning (pregnancy kits, ovulation kits)
  • First aid dressings & Supplies (band aids, non sports tape, etc.)
  • Hearing aid/medical batteries
  • Walking aids
  • Nebulizers
  • Wheel chairs
  • Reading glasses

Balance:

You may check the balance of your CentAccount Prepaid card anytime 24/7 by calling 1-866-809-1091, or by visiting https://www.mycarecard.com and clicking on Employee & Cardholder Log on.

*Members will only receive $50 reward for their annual physical if it is completed between days 91-365 of enrollment. Wellness visits within the first 90 days of membership with CeltiCare do not count as annual physicals.

** Please make sure to run your prepaid CentAccount Prepaid card as a Credit Card when making purchases.
